Findings In this cohort study of 25 315 women followed up for 25 years, higher adherence to the Mediterranean diet was associated with a 23% reduced risk of all-cause mortality. Biomarkers of small molecule metabolites, inflammation, triglyceride-rich lipoproteins, insulin resistance, and body mass index contributed most to explaining this lower risk, with only minimal contributions from standard cholesterol or glycemic measures.
Meaning In this study, higher adherence to the Mediterranean diet was associated with one-fifth lower relative risk of mortality, which could be partially explained by multiple cardiometabolic risk factors.
